Middle/High Menu Christian Catering Company Find the form you need from the list above and print it out. WRITE YOUR NAME AND GRADE ON THE FORM. Mark your choices clearly. Write your choices on a calendar or make a copy of the form for home. Send in payment with the form. Lunch forms turned in after the due date on the form will incur a $5 late fee! Students in the middle and high school will be told when their balance falls below $10. Elementary and Pre-School students will receive a note home in their Friday folders when their balance falls below $10. Students who have a negatice account will receive a sack lunch until their account is settled. Remember all lunches ordered will be charged to the student's account unless a parent calls by 8:30 am to cancel the order. 
Parent or Guardian must call 553-9630 by 8:30 am to cancel a lunch order when a child will be absent. |
In order to accurately count lunch choices, all forms must be turned in by the date posted on the form.
