Parents Guide to Holy Spirit Athletics
Holy Spirit and the AHSAA require specific forms/requirements to be filled out each year in order to participate in middle school, junior varsity, and varsity sports.
The following is a list of forms/requirements that you will be required to complete in order for your child to compete. Most of the forms are accessed through DragonFly:
- Holy Spirit Athletic Participation Form
- Preparticipation Physical Evaluation Form (front page of sports physical)
- Physical Examination (back page of sports physical)
- Birth Certificate
- Sportsmanship Course Certificate (NFHS Sportsmanship directions)
- Concussion Information Form
- Participant Agreement, Consent Release and Venue Form
- Sports Participation Fee (one time fee paid at the beginning of each school year)
